MHC Compliance Program:

This document outlines the scope of MHC’s Compliance Program, identifies key roles and steps to ensure an effective program is maintained at all times.


Uses and Disclosures of PHI:

This procedure specifies that the disclosure of Protected Health Information (PHI) is made consistent with applicable laws, regulations and health information standards, and to ensure that any disclosures of a Member’s PHI to family, other relatives, close friends or other persons designated by the member are appropriate.

Member Requests to Amend PHI:

This procedure ensures MHC maintains a process for responding to a member's request for an amendment to Protected Health Information ("PHI").


Notice of Privacy Practices:

This procedure ensures that a Notice of Privacy Practices is provided to each enrollee or his/her personal representative upon enrollment with MHC.

Safeguarding and Storing PHI:

This procedures provides guidelines for the safeguarding of Protected Health Information ("PHI") with MHC and to limit unauthorized disclosures of PHI that is contained in an individual's file, while at the same time ensuring that such PHI is easily accessible to those involved in providing service to the individual.


Internal Uses and Disclosure of PHI:

This procedure ensures that MHC’s uses and disclosures of Protected Health Information ("PHI") are limited to the minimum necessary to accomplish the intended purpose.


Privacy Complaints:

This policy ensures the privacy of Protected Health Information (PHI) as well as to ensure that such information is used and disclosed in accordance with all applicable laws and regulations.
